The day begins slowly as Weaver calls to say she'll be late, and Chen thinks she's developing a fever. Then multiple MVAs come in, quickly followed by two children suffering what appears to be smallpox. The staff panics, but not as bad as the patients, when a lockdown is ordered for the ER. The feverish Chen is quarantined with homeless Stan. Lewis takes charge as Weaver arrives and the police won't let her in. The girl infected by smallpox dies and Pratt embarasses Carter in front of her parents. Carter notices Pratt is sick as well, and sends him to Chen's room. Jerry phones the news, and the patients riot, nearly using a gurney to break open the emergency bay doors. Control of the ER is relieved from Weaver by an infection specialist. The staff prepares to stay, still not having a confirmation on whether it is smallpox or not, and the lockdown continuing as a result. Carter and Abby kiss after being quarantined together.

Pendant ce temps, Jing-Mei révèle qu'elle a examiné les enfants infectés une semaine auparavant et est donc placée en quarantaine avec Stan, un SDF. Et lors d'une dispute, Carter se rend compte que Pratt est fiévreux. Il est donc lui aussi envoyé en quarantaine.
Carter et Abby sont ensuite plaçés eux aussi en quarantaine en raison de leur exposition prolongée avec virus.
A la fin de l'épisode, ils se retrouvent seuls en salle de réa et Carter embrasse Abby en lui disant qu'ils s'en sortiront.
Un nuovo giorno al Pronto Soccorso: Chen ha la febbre. Questa si moltiplica e potrebbe essere collegata ad un disturbo che sta affliggendo due fratelli. Carter si convince che potrebbe trattarsi di vaiolo. Così viene ordinata la quarantena del reparto, mentre il panico tra lo staff ed i pazienti dilaga. La Polizia impedisce a Kerry di entrare nel Pronto Soccorso. Anche Pratt viene contagiato. Carter prende in mano la situazione, mentre Jerry fornisce di nascosto alcune informazioni ad un canale televisivo. Alcuni pazienti tentano la fuga ma John riesce ad evitare il peggio, mentre si attendono i risultati di un'equipe di specialisti. In attesa dei risultati e nonostante la situazione di quarantena, Abby e Carter si baciano.

Los residentes de Chicago son victimas de una epidemia. Cuando Carter y Abby tratan a dos jóvenes con los mismos síntomas, la sala de urgencias se declara en cuarentena hasta que lleguen los expertos del gobierno federal. Mientras tanto, Pratt y Chen, que pueden haber contraído la enfermedad, ignoran sus propios síntomas mientras intentan salvar la vida de un vagabundo, paciente habitual del hospital.
